Degenerative disc disease (DDD) is a broad term for age-related changes in the intervertebral discs, including desiccation, height loss, annular fissures, and endplate changes. MRI provides detailed evaluation of disc hydration (Pfirrmann grading), endplate changes (Modic classification), and associated findings. Pfirrmann classification grades disc degeneration on MRI: Grade I (bright white, normal height), Grade II (slightly decreased signal, clear nucleus-annulus distinction), Grade III (gray, unclear distinction), Grade IV (dark gray, decreased height), Grade V (black, collapsed disc space).
